As the old saying goes, fool me once shame on you, fool me twice shame on me. The same can be said for safety incidents. If one occurs on your jobsite it can be a tragic thing. That being said, with every experience comes a learning lesson.
Most safety incidents have a cause, effect and steps that could have been taken to avoid it, to document this to ensure that other teams and people within your project or organization can be learned it can be useful to prepare a lessons learned bulletin.
This document allows you to record details of the incident, the steps that were taken to cause it and what can be done to prevent it in the future to protect others. Once completed you can distribute it to the various groups.
